APPROVAL OF RENEWAL OF MANDATED PROVIDER AGREEMENT
BETWEEN REVCO DISCOUNT DRUG CENTERS, INC. AND KERR
COUNTY, TEXAS, BEGINNING OCTOBER 1st, 1988 THROUGH
SEPTEMBER 30th, 1989
On this the 12th day of September 1988, upon motion
made by Commissioner Holland, seconded by Commissioner
Baldwin, the Court unanimously approved the renewal
of a Mandated Provider Agreement between REVCO DISCOUNT
DRUG CENTERS, INC., and KERR COUNTY, TEXAS, beginning
October 1st, 1988 through September 30th, 1989.

Hospital and Kerr County, and between Sid Peterson Memorial
Hospital and all physicians (medical doctors) on staff at
Sid Peterson Memorial Hospital are due to expire September
30, 1988. If renewal of these Agreements is desired, please
forward a letter to us requesting renewal for the period
of October 1, 1988 through September 30, 1989.
