This is written notice of my intention of termination of
employment in your office.
The severeness of my billateral congenital hip dysplasia
is now the cause of billateral degenerative arthritis of
the hip joints which is greatly effecting the limitations
of my standing,•..a+~talking di.stan,ce:, bending, ,s.~AOpi,ng, .lifting
and carrying abilities.
I have been advised the situation will only get worse with
time and that a total joint arthroplasties of the hip joints
for pain relief and the improvement of functional capacity
will be required.
Due to this handicap disability and its restrictions I
feel I can no longer work at this particular job.
