(Jermiah O'Fihelly) Mr^. OrFihelly stated he has been with Kerr' County and available for two year^s as he had promised. He introduced Dan Schewe as the broker that p~_~t the proposed plan together far^ the renewal of ESG Health Glans contr^act. Mr. Schewew explained the renewal offer and compared it with the estimated PHO renewal. He explained the Kerr• Co~.inty benefit plan already contains a pr•nvision to waive the deductible for F'RGE 119 expenses incurred in Sid Peterson Hospital. It accomplishes utilizing a hospital with lower cost than hospitals in San ~1 Antonio and Bustin and allows the county to be a r^esponsible community citizen. Compar^ing the County to F'HO Physicians it is r-easonable to expect that the doctors will agree to a discount arrangement of same type with the discount passed dir^ectly on to the County in the for^m of reduced claims cost. A plan could be designed which would encourage utilization of the PHO pr•aviders and control aver•~all claim costs. It co~.~ld include three changes to claims incurred as a doctor copayment, 85Y.r-eimbursement or• deductible change. R copayment for doctor's of 115 would encourage utilization of PHO doctors. R possible negative effect of a doctor capay is is the distinction which must be drawn between routine outpatient doctor charges and r•adioloyy and lab charges. A possible negative of not having a doctor- copayment is the possibility that the doctor will require f~_ill payment at the point of service based on his standard charge and the claim will be reimbursed to the member based on discounted charge. The member would be in the position of having to recover the overpayment from the doctor^. Dan explained that HMO's ar-e locked in an ar^r•angement the differ-ence in being like gatekeepers. The employee has to see the primar^y doctor first and he recommends the doctor' to go ta. If Sid Peter^s on is used the deductible i:> waived. If use of another hospital you have to pay the ded~.~ctible. Most F'F'O's have access fee from 31.0~7J to 32. 5m a month. The PHO's should offer- their- r'~\ best deal to the county. They may ask for a presentation and ask for- a two year contract with the county so to lock you in. Either- a two year deal ar maximum increase for that period of time. Mr. Dan Schewe gave us some positive ways to do so. He has no problem with EPP and Jerry O'Fihelly has served our- account very well. I would like to explore ways to reduce our casts.
