B. Recognizing that a part of a client's rehabilitation program may include time away from the residential setting of the Service Agency, such as weekends, holidays, etcetera, and that the Service Agency must retain space for the client until his return; the KERB JUVENILE PROBATION DEPARTMENT will pay the Service Agency the agreed upon amount for such regularly scheduled days away from the Service Agency providing they do not exceed ten (10) days at any one time. Page 1 of 4 C. If a client makes an unauthorized departure from the Service Agency, the KERB JUVENILE PROBATION DEPARTMENT shall be notified immediately. If space has been retained for a client who returns to the Service Agency within ten (10) days, the Service Agency shall receive payment for those days the client was absent from the Service Agency, but not to exceed ten (10) days payment. D. The Service Agency is under no obligation to retain space for a client in unauthorized departure situations. III. A. Payment is to be monthly, or at the termination of the resident period of the client, if sooner. Claim for payment will be submitted by Service Agency no later than seven (7) days from the last day of the month for which payment is being requested. IV. A. The Service Agency is under no obligation to accept a client who is deemed inappropriate for placement in the program by the Service Agency. B. Each client placed with the Service Agency shall have a written Individualized Program Plan (IPP) developed in concert with the youth and mutually agreed upon by the appropriate Service Agency staff and the appropriate KERR JUVENILE PROBATION DEPARTMENT personnel prior to placement. C. The IPP shall contain the reasons why the placement will benefit the client and specify behavioral goals and objectives being sought for each client. Include shall be how the goals and objectives are to be achieved in the Service Agency placement. D. The IPP shall be reviewed jointly by the appropriate Service Agency staff and the appropriate KERB JUVENILE PROBATION DEPARTMENT personnel at reasonable intervals to assess the client's progress with modification of the IPP being made when indicated. Page 2 of 4 E. Copies of the original IPP and the periodic reviews are to be maintained by the Service Agency and the KERR JWENILE PROBATION DEPARTMENT. F. Although these periodic reviews may occur at different intervals, depending upon each client's particular situation, at least one such review shall be accomplished each ninety (90} days during the placement period. V. A. The Service Agency agrees that it will permit KERB JUVENILE PROBATION DEPARTMENT to examine and evaluate its program of services provided under the terns of this contract and to review client records. B. The Service Agency shall provide to the KERR JUVENILE PROBATION DEPARTMENT such descriptive information on contracted clients as requested on forms provided by the KERB JUVENILE PROBATION DEPARTMENT. C. A progress evaluation will be forwarded to the KERB JUVENILE PROBATION DEPARTMENT at the time of termination of services by the Service Agency. VI. A. The Service Agency shall be licensed by the Texas Department of Human Resources for those programs which are under the licensing authority of the Texas Department of Protective and Regulatory Services. B. The Service Agency shall meet all other required licenses pertaining to health, fire and safety standards. C. The Service Agency shall meet the KERB JUVENILE PROBATION DEPARTMENT standards for subcontract Service Agencies. VII. A. The Service Agency and Probation Officer shall make a reasonable attempt to seek contributions from the family to purchase clothing, personal articles and funds to assist in supporting the youth's rehabilitation. VIII. A. The contract may be terminated by either party by giving ten (10) days written notice to the other party hereto of the intention to terminate. Page 3 of 4 B. Termination of a student's residence with the Service Agency should occur only after notifying the KERR JUVENILE PROBATION DEPARTMENT of the alternate placement. IX. A. The Referral Agency, KERB JUVENILE PROBATION DEPAR'I'MEN'f, understands that the Service Agency will not be responsible for medical and dental costs incurred. The Referral Agency, KERB JUVENILE PROBATION DEPARTMENT, agrees that it shall be liable and responsible for the client's medical and dental costs encumbered during the client's stay at the Service Agency. In the event that the Referral Agency, KERR JUVENILE PROBATION DEPARTMENT, cannot assume liability for the client's medical and dental costs encumbered during the client's time in residence, the Referral Agency agrees to access whatever funds are available for these costs through the client's parent's insurance or other eligible government funds. The Service Agency agrees to notify the Referral Agency, KERB JUVENILE PROBATION DEPARTMENT, of any unusual medical costs to be incurred. X. A. Specifically, the Center promotes the social and economic independence of disadvantaged youth, 16 through 17 years of age who are adjudicated and at risk of lifelong dependency. The mission includes a campus based service delivery system that provides assessment, vocational skills training, youth competencies training, remedial education, GED preparation, vocational counseling, driver's education, occupational orientation, a residential program, casework and peer group counseling, work experience and job placement. The Center is accredited by the Southern Association of Colleges and Schools (Commission on Occupational Educational Institutions), licensed by the Texas Department of Human Services to provide basic child care for 160 youth and approved by the Texas Education Agency- Division of Proprietary Schools. The Center receives funding from the Houston Job Training Partnership Council for a portion of its training and work experience component. Partial funding for the educational component is also provided by the Texas Education Agency through the New Waverly Independent School District. Funding for residential services is also provided through contracts with the Texas Youth Commission, county juvenile probation departments and the Texas Department of Protective and Regulatory Services. The Center is located in the heart of the Sam Houston National Forest. The main campus is on approximately 46.7 acres in Walker County, Texas, four miles west of New Waverly and fifty-five miles north of Houston. An additional campus located in north Montgomery County is the site of the Center's transitional living program. The Independent Skills Learning Environment (ISLE) is designed to provide students with opportunities to live in a simulated independent living environment in which they can put into practice transitional living skills learned in the classroom. Vocational training is offered in automotive mechanics, building maintenance, carpentry, general construction, trowel trades, business office services, painting and decorating, horticultural related services and food services. Remedial basic reading and math subjects are taught using individualized and computer assisted methods. Students are also offered the opportunity to earn a GED certificate and a Texas driver's license while at the Center. All vocational courses are TEA approved. The opportunity for practical application of skills learned is offered to students through the work experience program. During this phase, students work in trade related jobs either on campus or with community service organizations off campus. Students are paid the minimum wage which is held in an account for the student to be disbursed after graduation. This money enables the student to purchase tools, transportation, housing or other support needed when they begin employment in the community. A caseworker is assigned to each student to ensure that a plan of service is developed to meet individual needs and that it is reviewed and followed as the student progresses through the program. The caseworker provides advocacy, counseling, crisis intervention, case management, family contact and overall program coordination for the student. Support is provided for each student in the form of needed medical services, clothing and personal care items. Students are also provided transportation for a home furlough on a regular basis. Students who have experienced substance abuse are referred to counselors who provide multi-faceted support in this area. This program includes specific group and individual counseling, educational programs and Self Challenge, a program of physical experiences oriented toward developing self esteem, confidence and team support. The average length of stay is 6.5 months. Graduation is determined by social adjustment and competency attainment. During most of this time the students live in small groups in a dormitory setting. Groups are based on trade classes in which youth can develop positive relationships, effective problem solving skills and constructive peer support systems. Group sessions are held each weekday supervised by residential services staff. Students are encouraged to discuss their daily activities and progress as well as to seek support in working through any problems they may be experiencing. Group counseling is geared toward preparing the student to meet their individual future goals and objectives. Students participate in a level system which is based on accepting responsibility for one's own behavior. The system is designed to reward self discipline and positive example. Rules and consequences are written, given to, and discussed with students at admission and reviewed as needed. Recreational activities include such sports as football, softball, basketball, weight lifting and swimming, as well as other games. Students may also become eligible for off- campus recreational activities. Recreational activities are targeted toward meeting the social and physical needs of the student. Throughout the program, students are given training which will prepare them for successful employment and independent living. However, during the work experience phase of the program, students may participate in the Center's transitional living program at the ISLE campus. In this setting, students live with a roommate and exercise greater independence and responsibility for their own care and actions. Additional training is also offered in a variety of independent living skills. After graduation, students are assigned to specific Community Services Specialists who assist the youth during their transition from the institution to the community. This includes job placement and referral services. CLIENTS SERVED Students at Gulf Coast Trades Center are adjudicated 16 and 17 year old LOC 3 and LOC 4 youth who express an interest in vocational training and have demonstrated an ability to benefit from the training. Since 1971, the Center has served over 10,000 adjudicated youth. ADMINISTRATIVE STAFF Thomas M. Gulf Coast Trades Center has been recognized by a number of national and state agencies for the quality of its service delivery and its commitment to the needs of the juvenile offender. SERVICES Services to be provided under this referral agreement will include intensive assessment, evaluation and case management services delivered in a multi- tiered system utilizing 2 separate campus facilities and includes the provision of community based aftercare and follow- up services designed to ensure the graduate's successful transition to the home and work place. Students classified as LOC 3 and LOC 4 who have been determined to have the motivation and social skills necessary to participate in the residential vocational training program will be enrolled at the New Waverly Campus. Here they will begin their vocational and academic studies with their goal being vocational competency attainment and upgrade in academic skills functioning. Based on behavior level and vocational skills acquisition, these students may also become eligible for residence at the Willis Campus. Students generally become eligible for consideration for this program after their third month in residence at the New Waverly Campus. The Willis Campus offers opportunities for independent living situations in supervised apartment living and subsidized work experience with local area non- profits and governmental entities. These work experience positions allow the student to display their newly learned vocational skills as well as providing meaningful community service. Based on evaluation by Department and Center staff, LOC IV students will participate in an intensified assessment and evaluation program. Typically, students enrolling in this tier of services will have participated in more serious negative behaviors, be in need of more in depth evaluation and counseling, and will have a higher incidence of negative behavior than other LOC 3 generally referred to the New Waverly Campus. The services offered are consistent with the service requirements of Level 4 youth and involve a multi- discipline approach to evaluation, therapy, recreation, physical activity and counseling with the goal being to determine the student's likelihood for continued study in vocational training or independence in the community. The following services are funded, in part, by the daily rate charged to the referring agency: *Intake assessment and orientation, *Casework services, including development and monitoring of a plan of service, counseling and substance abuse services, *Behavior modification through a structured level system, *Education in vocational skills, related academics, and GED preparation, *Driver's Education, *Tranning in pre-employment, work maturity and independent living skills, *Residential care, personal care items, clothing, *Recreation activities, *Transportation, *Work experience and job development services *Aftercare and community based transition services. AUDIT The Center will assist the County Auditor in any review or monitoring of fiscal procedures and methods as stipulated by contract. STANDARD CONTRACTS Signed copies of standard contract forms are attached as a part of this submission. YOUTH SERVICE GOALS Gulf Coast Trades Center has demonstrated proven success in preparing its students for employment and independent living through general and specific vocational training, remedial academic achievement, GED preparation, driver's education and counseling and activities designed to prepare youth for fulfilling the responsibilities of being a good citizen. The Center serves LOC 3 and LOC 4 youth and designs individual plans to address the needs of each student. Educational achievement and employment are two of the main goals for each youth in residence. The Center maintains high rates of success in both areas. The overall employment rate for Center is graduates is 59%. For those students who attempt the GED, the success rate is 62%. The ultimate objective for each youth enrolled at the Gulf Coast Trades Center is best stated in our mission which is "...to increase the social and economic independence of disadvantaged youth." LICENSES/ APPROVALS Attached are copies of the current licenses granted to Gulf Coast Trades Center. FEES In consideration for the above referenced services the Center proposes an amount not to exceed $58 per clientlday for youth identified at Level III (Intermediate Level of Care).
