mentioned property. At this time, our records indicate the Warranty for this project has
expired. To renew the Warranty on an annual basis, the cost will be $675.00 per year.

and return it to us with your check for the amout indicated above.

Kezar's Pest Control, Inc., provided the necessary pest control treatment to property
identified as specified:
Pre-Construction Subterranean Termite Treatment
Post-Construction Subterranean Treatments

WARRANTY RENEWAL
Service warranty renewal is subject to the specification on the original Warranty
submitted and the general terms and conditions as listed therein. Kezar's Pest Control,
Inc., will make an annual inspection of the premises at the beginning of the Warranty
Renewal period at no extra cost. Kezar's Pest Control, Inc., will apply any necessary
additional treatment to control any subterranean termite infestation discovered during
the period the service warranty renewal is in effect, at no extra cost.
1. Services performed by Kezar's Pest Control, Inc., will be conducted in a manner
consistent with that level of care and skill ordinarily exercised by members of the
professional currently practicing under similar conditions. No other warranty,
either expressed or implied, is made or intended by our proposal or contract.
2. If live infestation of termites are found when a break in the chemical barrier or
foundation has occurred during the period of this agreement, Kezar's Pest
Control, Inc., will reapply any necessary treatment during the period that this
agreement remains in full force and effect. This agreement shall be in force for
one (1) year from the renewal date at $675.00 per year.

3. Water Leakage -- Structural or mechanical defects which result in water leakage
in interior areas or through the roof or exterior walls of the identified property
may destroy the effectiveness of Kezar's Pest Control, Inc., treatment. Such
leakage can further produce unusual situations abnormally conducive to new
infestation. If such condition is discovered, it is agreed that the Owner will be
responsible for making such repairs as are necessary to correct said defects and
for repairing any damage which was caused by said water leakage and/or
termite infestation allowed to continue as a consequence of such leakage.
Kezar's Pest Control, Inc. will upon completion of repairs necessary to correct
said defects, provide additional treatment to control infestation in the area. The
cost of said additional treatment will be paid by the Owner.
4. In the event the premises are structurally modified, altered or otherwise changed
(including landscaping), from its original condition after the initial treatment, this
agreement shall terminate unless treatment is applied to those areas which are
structurally modified, altered, or otherwise changed. The cost of said additional
treatment will be paid by the Owner.
