FROM: Architect -Michael N. Walker August 21, 1998 The following list is a compilation of items observed by the Architect and Lux Engineering on awalk-through on August 20, 1948. This list is hereby attached to the Certificate of Substantial Completion. Final aceep- tance by the Owner is dependent on the Contractor successfully rectifying items on this list and is subject to a fmal observation, which will be performed by the Architect and Engineer. Rectification of items on this list does not relieve the Contractor of other specific requirements of the Contract Documents. Architect is rec- ommending that the Contract Time be extended from the original Completion Date of August 21, 1998, two weeks for delays to complete surveying, till September 4, 1998. Such change will be reflected in Change Order #2, which is delayed pending costs from Contractor for credits, as further noted herein. GENERAL: 1. Retum all keys to Owner which aze in the possession of Contractor, including Contractor installed locks. 2. Comply with applicable requirements of Section 01700, including providing: a Certificate of Occu- pancy, one year Warranty, detailed List of Subcontractors, proof of payment, Releases of Liens, and thoroughly mazked Job Set of Construction Documents with changes in discovered conditions and changes in the Scope of Work (famish to Architect to transfer to final Record Documents). 3. Complete patching and clean-up when all of the corrected work below is complete. 4. Remove concrete debris from ledge at exterior wall down to structural slab. 5. ConduiUwiring to previous Kitchen panel is stubbed through floor; re: Note E4. 6. Not as per Contract, Contractor removed from site stored water closets and lavatories. For reference there were at least 3 (possibly 5) re-usable water closets and 3 lavatories, which list in American Stan- dard for about $300 and $200, respectively. Contact Lux Engineering for an approximate value of the water heater not remaining as noted. credit due Owner to be reflected in Change Order #2. Owner decided that due to sewer gas problems not to take the $1,500 credit offered to Owner for Con- tractor not completing removal of pipe and grouting openings in floor per Spec. Odor problems oc- curred again, on August 21. Southeast quadrant ceiling -remove hanging flex conduit; cap wiring in junction box at roof deck. 9. Replace 8-]0 damaged and missing ceiling tiles in Tax Office (see County Maintenance Director). 10. Obtain costs of leak damage from County Auditor to add to proposed Change Order #2. ELECTRICAL ROOM (Use licensed electricians where appropriate) 1. Remove remaining control wiring and conduit from 4x4 gutter and to above concrete ceiling. 2. Remove abandoned conduit and circuits above Control and Electrical Rooms ceiling levels. 3. Remove lighting fixture from wall to the left of entry to Electrical Room, which was Toilet Room. 4. Remove abandoned fire alarm devices on right wall of entry to Electrical Room. 5. Remove switch box and conduit to the left side of ElectricaUControl Rooms door. 6. Remove 1" conduit, junction box, and control wiring from above stairwell and out into open space. Remove (or explain why not removed) Toilet Room side of plaster chase wall behind Electrical Room. Remove and or cap per Specifications and previous instructions all plumbing and electrical; do not remove plumbing vent which passes through from First Floor to roof. 8. Panels C and EM in Electrical Room: Remove all non-functioning wiring and conduit (as a minimum disconnect wiring from breaker and safely coil), replace covets, label clearly all remaining circuits, and clean space and panels. EXTERIOR -ROOF (Services of a professional roofer are strongly recommended) Caps on removed HVAC units are not acceptable. Remove all duct tape and poly sheeting. Replace with base flashing or roll roofing specifically designed for that purpose. Tum-downs on sheetmetal caps are fabricated too tight and short to counterflash over base flashing; re-fabricate and/or devise an acceptable method to counterflash over the exposed edges of existing (and/or added) base flashing, which must be positively lapped under the sheetmetal caps or acceptable patching. If caps are re- fabricated, include hems per Specification. 2. Some of the mechanical curb closed opening do not appeaz to have splice sheet of plywood over top, as discussed, for better support? 3. Rain is leaking inside below where facia was removed and replaced. Roof patching is rejected at patch over facia blocking. Replace with base flashing or roll roofing specifically designed for that purpose. 4. Rain is leaking in at ~b of the removed plumbing roof vents and dryer vents, onto the floor and into the soil pipe below, and probably eventually into the spaces on the First Floor due to the sanitary piping Kerr County Second Floor Annex Demolition Cortections List -Page 2 of 3 being wt and loosely capped below. Cap each vent projecting through roofing from which piping was removed, with an appropriately sized exterior grade rubber cap and hose clamp. 5. Two remaining rooftop I-IVAC units shall be tested for heating and wolfing with operation controlled from 2 thermostats added for that purpose. North unit has long-standing gas leak; gas valve is wt off for safety. If fan (coil) units and compressors and gas heat can be demonstrated by Contractor to operate at all, Owner will arrange for further service. It is acceptable to leave exhaust fan motors in existing housings; diswnnect and remove abandoned conduit, wiring, and devices. 6. Clean roof of fasteners, debris; patch damaged azeas of roofing surface. Replace missing roof hatch handle. TEMPORARY HOLDING CELL Replace screws in Vestibule and Cell fixtures and added cover plate with appropriate size tamper proof. Clean lens of Cell fluorescent fixture. 2. Make bubbler more functional; possibly try adjusting from chase behind and/or clearing the line with compressed air or water pressure. 3.
