ASSISTANCE ORGANIZATIONS Non-profit organizations that receive state funds and provide educational,
health, or human services orprovide assistance to homeless individuals
Documents required:
- Board approved resolution
- Articles of Incorporation and Certificate of Authority. A letter from the Secretary
of State with the entity's charter number evidencing that the entity filed for
incorporation will be accepted in lieu of a certificate of authority. GSC Co-Op
will not accept By-Laws in lieu of Articles of incorporation
- Current contract or grant from a State agency to prove State funding. This.
document must show beginning and end dates and these dates must be valid
at the time the application is reviewed.

I am writing you after several failed attempts m call your office regarding your account with the General
Services Commission Cooperative Purchasing Program (GSC Co-Op). The phone number we have on file
for you, 830/896-1337 rings and no one answers. Believing I had xn incorrect number, I called directory
assistance and they gave me 830/792-2235. When I dialed it I got a message telling me that this is not a

so I can update our files with your correct phone number.
I received your renewal payment on Apri129, 1999. However, due to an audit of your file, GSC Co-0p
reyuires the following information before we can extend your membership agreement
