Attachments Without Forms Some required attachments do not have accompanying forms. Following are each of these along with the circumstances under which it must be submitted. Use the checklist below to determine which attachments you must submit. ^ Resolution -Except for applications from state agencies, each application must include a resolution from the applicable governing body, such as the city council, county commissioners' court, school boazd, or board of directors, that contains the following: ^ authorization for the submission of the application to CJD that clearly identifies the project for which funding is requested; ^ a commitment to provide for any applicable cash match (if the applicant is not providing all of the matching funds, then the other participating entities that aze providing portions of any cash match, must also submit resolutions -this does not apply to state agencies); ^ if the applicant chooses to designate an authorized official who is not the chief executive officer, the resolution must provide the designee with the power to accept, reject, or alter a grant; and ^ a written assurance that, in the event of loss or misuse of CJD funds, the governing body will rerurn all funds to CJD. ^ Cooperative Workittg~greement - When an applicant intends to carry out a project lazgely through cooperating with outside organizations, the applicant must obtain authorized approval signatures on the cooperative working agreement from each ouzside participating organization. Non-juvenile program grantees under the Criminal Justice Planning Fund (Fund 421) must provide CJD with a signed copy of any such agreement(s). ^ If the application is for amulti-jurisdictional task force, then a cooperative working agreement must include each chief of police and each sheriff in amulti-jurisdictional task force's impact area. Counties must be contiguous and the chief or sheriff may not execute a cooperative working agreement with more than one task force project.
