The purpose of this list is to provide a Substantial Completion Certificate and acceptance for occupancy for this portion of the Project: partial Phase 3b -the balance of interior of Second Floor of the Courthouse. The Contractor furnished a list of remaining completion items. Any portion of the Project deemed by the Architect as not ready for inspection is so designated and subject to a subsequent inspection. The following list is an attachment to a partial Certificate of Substantial Completion for the Second Floor Courthouse interior spaces only, which items must corrected, or if not practically correctable and agreed as acceptable by the Owner and Architect, a credit be issued to the Owner. Final acceptance of this partial phase by the Owner is dependent on the Contractor successfully rectifying items on this list and is subject to a final observation, which will be performed by the Architect and Engineer(s), and a Final Certificate of Occupancy issued by the City of Kerrville. Rectification of items on this list does not relieve the Contractor of other specific requirements of the Contract Documents. Contract Time was extended to February 24, 2001, by Change Order #3. GENERAL ITEMS: 1. Ductwork must not interfere with ceiling suspension system or vice versa. Suspend duct from structure per Specifications (not from other ducts, piping, or compromised plaster ceiling). 2. Suspension wires for ceiling must be vertical, near vertical and/or balance tensioned if not vertical, do not allow wire to bend around ductwork. Do not kink wires to achieve level grid. Pull all grid suspension wires into tension. Poke-through-plaster-grid-section anchors for ceiling grid suspension wires are rejected. Anchor to structure above or to existing 1-1/2" channels (not 3/4" channels). New suspension wires need to be individually tested. Remove unused suspension wires. 3. Trim lath at openings and access holes to prevent injury and improve accessibility. 4. (e.g., abandoned radiator lines, water piping, conduit, ducts). Remove trash, demolished plaster, the cut-offs pieces, and excess construction materials from crawl space between structure and plaster ceiling, as a minimum trade off for not complying with Note R11 to remove existing plaster ceiling. 5. Isolate and suspend flex duct to prevent damage from wire lath and other protrusions, and to allow access without damage to duct. 12 wide gypsum board underlay is acceptable, if suspension is not possible. Trim away lath from flex duct drops. 6. Fire caulk all penetrations of fire wall. 7. Could not check above existing plaster ceiling due to lack of ladder of sufficient height; therefore, all suspension, cleaning, mechanical and electrical were not inspected. 8. Adjust all closers to ADA/TAS pull/push pressure 9. Remove all smoke alarm covers. 10. Fully insulate all water piping above ceilings. Per Specification seal joints with mastic and tape as recommended by the insulation manufacturer. 11. Provide air balancing data as specified for review by Engineer. 12. Exchange the two differing style thermostats on the Second Floor with two of the thermostats located on the Lower Level non-public area of the annex so that all thermostats used in the public areas of the Courthouse are of the same type and style. 13. Provide Owner's Operating and Maintenance Manual to the Architect /Engineer for their review and approval prior to submittal to the Owner. 14. Since the weather did not allow adequate checking of the air handling units during cooling conditions, water test drain pans, condensate and auxiliary drains to assure leak proof and properly draining pans and piping. Notify architect and engineer of time such a test is to take place, for their possible observation. SPECIFIC ITEMS: LAW LIBRARY: 1. Resolve AHU-1 accessibility problems and safety issues and walking on existing plaster ceiling, which Contractor chose not to remove, contrary to Contract Documents. 2. AHU-1 -clean out and water test drain pan; better support refrigerant and drain lines for entire distance; slope lines for positive to drainage; supply duct support is lacking; unit has gap that allows attic air to enter return end -provide sheet-metal closure. Excess return duct in flex duct may remain, if properly supported. Replace 2 return air registers that are not per Specifications -replace. Replace flex duct to Toilet 4 exhaust vent with specified material. 7. Suspended ceiling - 4 hanging wires missing, broken loose, or tied to 3/4" channels along west wall; wires missing at AHU-1; suspension wire conflicts with duct (re: General above); wires are not tight near center of room. (could not verify] 8. Ceiling is low in south end (under AHU-1) and above Door X08 at light fixture. Ceiling grid is crooked; cause under study (credit?). 9. Conduit to exit light should not be exposed Wiremold; it should be conduit inside new wall. 10. Close opening behind counters and shelving below window sills; trimmed urethane foam is acceptable under counters where not exposed to view; matching sealant may be used above counter to close ends at sills. 11. Fill gap in wall at telephone or computer cover plate outlet near Door X08. 12. Existing window refinish defects: most appear to lack second coats on upper jambs; most appear to lack cleaning and adequate sealer on top of meeting sashes; XA08 and XA11-fill holes in sill; XA07 remove weatherstripping nails; XA03 fill holes. 13. Open crawl opening between Law Library and Storage 2 above plaster ceiling by cutting away lath and channels (safely). 14. Edge of counter needs final smooth coat. Shelving also; seal tops of units. 15. Paint front of plaster base (after patching) at elevated floor step-up to Door X12 a dark enamel to match base. Same required for Toilet 3 Door X10 per previous Corrections List. 16. Many exposed nails on back of "U"-shaped shelving unit need countersinking and filling. FOYER 1. AHU-3 above ceiling was not given inspection before cover-up: a. Remove radiator pipe that is distorting duct and hampering proper air flow. b. Cut off end portion of uni-strut bar support extending over space planned for access to the air handling unit. c. Fasten duct, between fire dampers and unit, more securely than with the thin wire used. Relocate fire alarm wiring in access opening and AHU-3 to provide adequate access to attic and air handler for service. 5. Condensate drain connected directly to plumbing system with hose in Men's Room 3 needs a minimum 2" air gap at the connection to plumbing system. The present connection also fails ADA clear space requirements for handicapped access. Replace flexible pipe at the lavatory with a PVC or hard copper connection run directly to the wall then over to where the 1" pipe comes out of the wall. Separate the condensate pipe and add a funnel drain just above the plaster ceiling and chase. Provide a 2" gap between the condensate drain and this funnel with a fastener to keep the condensate drain directly above the funnel. 2. Top rail of toilet partition lacks blocking; attached to gypsum board (credit). 3. Close gaps around door frame trim with caulk; remove paint from trim. 4. Remove paint from top of base, trim. 5. Window XA14 blind missing tassel. 6. Could not verify Change Order 8 vent fan items were complete. Fan does not cut off. JANITOR ROOM NO. 2 1. Condensate drain installation as presently terminated in mop sink will compromise the use of Second Floor West Wing Courthouse -Substantial Completion Inspection Corrections List -Page 4 of 7 the sink. {nstall a P-trap and drain adjacent to sink into P-trap clean-out and terminate drain into it, leaving a 2" gap between condensate pipe and drain pipe. Fasten condensate drain firmly to avoid its dislocation and flooding from the condensate drain. 2. Touch-up finishes around incomplete mechanical installations. Clean room for inspection. 3. Trim off approximately one more foot of plaster ceiling evenly with a straight edge; trim lath to provide a smooth edge to avoid entry for access. WOMEN 3: 1. Drain line piping through fire wall needs better support. It might be advisable to consider a different routing of AHU-1, AHU-2, and AHU-3 drain lines to consolidate into a single funnel drain at an accessible point above this Toilet Room; Engineer Lux will contact affect mechanical subcontractors and city Building Official to resolve. 2. Ceiling grid is crooked; cause under study. 3. Fully insulate new and existing water piping; tape and or adhere insulation tightly to pipe. 4. Provide better separate support for exhaust duct. 5. Insulation installed on piping located in the Stair 3 needs to be properly installed by replacing the tape coming loose with tape recommended by the insulation manufacturer; also insulation to existing connecting 1" PVC drain damaged during installation. ROOF 1926 COURTHOUSE 1. Patch and match damage from this Contractor's Work. 2. Electrical closet at top of stair needs new directory for Panel S. 3. Patch holes on walls where plumber removed drain pipe. 4. Chem-curbs roof penetrations without indicated hoods are not going to be acceptable, especially at refrigerant roof penetrations. 5. Support conduit and refrigerant piping off roof deck. 6. Refrigerant has leaked on roofing, possibly damaging same. 7. What is problem with Phase 1 condenser which has exposed wiring (disconnected)? 8. Where are plumbing vent roof penetrations? Engineer Lux needs to view connections. 9. Plug holes in brick wall from piping, conduit removed with this Work (west wall Penthouse). 10. Cleaning, sealant work not started on existing cast stone joints. 11. Certification from roofing company and roofer for base flashing at existing penthouse walls (less than 8") is needed. 12. Remove trash. 13. Roof hatch is not spray painted inside and out with 2 properly applied coats; rushed coating and lack of prepared surfaces will not be acceptable. Do not paint gasket.. Offer Credit for poor and etched new and existing concrete at walks, curbs, and drives. RC. 8. Paint alt pre-primed operable window frames and sashes while open and/or off quick clip hinges; paint fronts, frames, and edges back to final weatherseal; mask all weatherstripping and hartlware; re- prime all exposed wood after sanding and re-priming to compensate for extended exposure to weather. 9. Replace or offer credit for cuts to window weatherstripping as a result of not opening windows to paint them. RC. 10. Most windows need sealant and paint at bottom sloped joint of vertical joints of brick molding and lower portions of jamb, and additional coat to window sill to compensate for extended exposure to weather. Brick mortar has an excessive spalling problem, which the Owner may be arranging to have tested. NRC. 19. Paint is sloppily applied on antl off of sealant joints; cut lines between sealant and paint are unac- ceptable; recommend solution or offer credit. 20. Sealant and asphaltic wall flashing, dampproofing were not installed per Contract Documents (in contact and near exposed surfaces) mostly around windows and doors. This prohibited contact has caused discoloration of sealant and will probably lead to failure of the joints. With the brick installed it is impractical to reach into the 1/4" to 1/2" wide joint and cut the wall flashing back the necessary 1/2" depth to re-work and replace the sealant joint. Building up of the sealant to achieve a 1/2" separation by over-coating is not advisable; therefore, other remedies are suggested. Chemrex (Sonnebom) representative, Don Hendrix, recommends the use of separation coating -Chemrex CX Pro 2000 acrylic. After curing apply a solid 1/4" of NP1 or NP2, or Owner may accept as a credit for work not properly installed. RC. SPECIFIC ITEMS EXTERIOR WALLS 1. Repair peeling paint at northeast comer of concrete wall; spot test other areas for adhesion. Verify specified coating and prep is used. If not per Specification, then remove and replace all coating with that specked. 2. North wall west end octet: provide missing nuts at water proof convenience outset and trim excess bolt length like other outlet; sealant ends . 3. Remove paint and repair as required on EIFS band at southwest comer lower level. 4. Trim area drain piping straight; approximately 1 inch exposed in each lower level retaining wall. 5.
