Came to be heard this the 28th day of March 2005 with a motion made by Commissioner
Letz, seconded by Commissioner Nicholson. The Court unanimously approved by vote

This agreement is made on ttris Qrao!~+day of ~~jr and between Dr. Lira M. Watts,
Llcerrsed Clinical Psyctalogist, and Kerr County Juvenile Detentiar Facility, herein after referred b as
('Contractor').
Whereas, Dr. Lisa M. Watts is a Licensed Gklical Psychabglst, herein after referred b as psychologist,
and is suitable to administer psychological evaluation and psydrottrerepy services fa at~isk juveniles;
Now, Therefore. the parties agree as foYows:
ARTICLE 1
1:01 Purpose: The purpose of the Contract is b provide psychological evaluation and consultation for at-
risk youth referred 6otn the Juvenile Detentrat Facility on an as needed basis.
ARTICLE II
2:01 Terrn: The tens of this contract is xx a one year period oommertcing on date and may be terminated
by either party with 24 Frorrra notice. If the Contracting Fadligr determines Drat Dr. lJsa M. Walls has

ARTICLE III
3:01 Services: Dr. Lisa M. Watts agrees b administer and provide psyrhotogical evaluations to youth
referred by the Contracting fatality. Furtirerrrrore,1118 Psyrthologisl agrees b administer the evaluation al
the earliest convenience at trey discretion. Suicide risk assessments lake priority over other services.
3:02 Reports: Dr. Lisa M. Watts agrees b provide a Dopy of the Psydwbgical Evaluation b fhe
Contracting Facility within 2 weeks of a juvenile incarcerated,and within 4 weeks of those not krcarcerated.
Suicde Risk Assessnlrarts will be provided within 48 hours.
ARTICLE N
4:01 Fees: The Contracts shall pay the sum of tourfiundred dolars and .00!100 (E400.00) per
psychological evaluation administered and e~hty--five dollars (385.00) per suicide risk assessment, to Dr.
Usa M. Watts. Furthemare, the Contracts agrees b pay Dr. Lisa M, Watts the sum ofone-hundered
twenty-five dollars and 901100 (3125.00) per appointment carrcellatton a `no show', unless prbr notice is
provided.
4:02 Billing: Dr. Lisa M. Watts agrees b tumish Contractorartitemized-bill, which shah kttdrbe the
charges spacafied above, and the names and the dates youths were eveluahed.
4:03 Payment: Payment shall be made within 30 days after receipt by Contractor, of Dr. Usa M. Walls
invoicing.

5:01 Entire Agreement: This agreement supersedes any and ah other Agreements, either oral a in writing,
between Itte parties hereto with rasped to the subject matter of this agreement.

6:01 Applicability: This Agreement shah irwre solely to the benefit of the parties hereto and not any third
party reapients.

unenbrceable prevision has never been contained herein.
