Came to be heard this the 28th day of July 2006 with a motion made by Commissioner Williams, Seconded by
Commissioner Nicholson, the Court unanimously approved by a vote of 3-0-0 (Conunissioner Letz. absent) to
pay for one months' premium of COBRA expense of $515.10 and a one time amendment to the administrative
service agreement with Mutual of Omaha.
