Schedule of rates Rate will be on an hourly bases at $100 per hour estimated hours to complete the project is 20 - 40 hours. Total fees not to exceed $4,000.00 with a minimum fee of $1,000.00. Minimum fee is due at time of Alpha delivery, see page 5 "Acceptance and Authorization". Payment terms Payment of all fees will be 30 days from the date of acceptance. 3/23/2009 Statement of Work Page 2 Statement of Work Statement of work Assumptions Some assumptions made by the contractor include all required documents are introduced at time of development acceptance. Access to end users will be available at the agreed upon times, to the consultant, for interviews and product validation. All workflows and processes to be included are disclosed at the time of development acceptance. Change management process All changes to the developtment document will be included in a change request and agreed upon between the contractor and the client. Frequent changes will or could increase the cost of production. All costs related to change requests above an beyond the intial program documentation will be at the stanard rate of $100.00 per hour and could increase the maximum fee of $4,000. The contractor agrees to notify the client in the event the maximum fee is capped. Engagement related expenses All engagement related exenses will be covered in the hourly rate imposed with the exception of any hardware requirements, if needed. Hardware requirements will be the responsibility of the client to procure, with the assistance from the contractor, if needed. Professional services agreement Scope of Work: The consultant will be engaged to complete an Access program to monitor, update, and produce word dacuments for submition to the county court. The program will replace the existing two phase approach to document completion through a Microsoft Excel and Microsoft Word document merge and the current Odessy system. The product will complete all documents needed for the court in one system versus two separate systems. The development will be preformed in phases using the System Development Life Cycle (SDLC) approach which includes the following phases requirements, validation, training and ownership. Monitoring I. Requirements The requirements phase consists of information gathering and analysis, design and building. 1. In the gathering and analysis phase the consultant will complete the following objectives. Conduct interviews with the end users to discuss their work flows, field requirements, documents and forms requirements. Additional interviews may be necessary to resolve work flow issues or to clarify requirements. 2. In the Design phase the consultant will work to complete the alpha version of the application and present the intial design to the County Attorney. After acceptance of the Alpha' version a Beta" version will be completed and additional testing and acceptance will be conducted. The Beta version is considered the final version and application acceptance is preformed at that time. II. Validation The Validation stage consists of testing on both the Alpha and Beta version of the applcation with a high degree of user feedback to determine changes, adjustments, or alterations to the product. III. Training Training will be conducted to all end users after acceptance of the product. Training may run in conjunction with the Alpha and Beta releases to check for quality of product development. Training may also be conducted fqr one power user employed with the client for desimination to other users. IV. Ownership The County Attorney will retain all rights to the application including source code. The client, at their descretion, may retain the consultant on an hourly or annual basis for the purpose of updating, modifying, or maintaining the product and data. 3/23/2009 Statement of Work Page 4 . °, Statement of Work Acceptance and authorization Product acceptance will be a two phase approach, Initial acceptance is deemed to be agreed when the the Alpha version is delivered. Final acceptance is deemed to be agreed when the Beta and or final release is installed and operational on all end user systems. The terms and conditions of the Professional Services Agreement apply in full to the services and products provided under this Statement of Work.
