TO OBTAIN A PASSPORT:

REGULAR SERVICE - Allow 6 to 8 weeks for your passport to arrive

EXPEDITED SERVICE – Allow 3 weeks for your passport to arrive.

WHAT YOU NEED:

1. Complete an Application form – may be obtained from the district clerk’s office

or online. Bring the form to the District Clerk’s Office.

DO NOT SIGN THE FORM-IT MUST BE SIGNED IN CLERK’S PRESENCE.

2. You will need 2 passport photos, they must meet certain specifications.

3. > Bring a certified copy of your birth certificate, WITH A RAISED SEAL; or

> An Original naturalization certificate for Naturalized citizens.

> An old passport may be used to prove citizenship on adult passports in lieu of

a birth certificate.

4. Bring a photo ID - Driver’s License or a student identification card.

PAYMENT We accept checks, money orders, cash or credit cards

REGULAR SERVICE:

Fee payable to Robbin Burlew, District Clerk is $25.00 + local processing $5.00

Fee payable to ‘US Dept. of State' is $75.00 for an adult 16 and over,

and $60.00 for a child 15 and under.

PASSPORT CARD:

Fee payable to Robbin Burlew, District Clerk is $25.00 + local processing $5.00

Fee payable to ‘US Dept. of State is $20 for an adult 16 and over,

and $10.00 for a child 15 and under.

EXPEDITED SERVICE:

Fee payable to Robbin Burlew, District Clerk is $46.25

Fee payable to ‘US Dept. of State’ is $151.25 for adult 16 and over

And $136.25 for child 15 and under.

• All applications for children under 14 require a certified copy of birth certificate.

• All applications for children under age 14 require both parent’s signatures.

• Do not sign your application until you are in the clerk’s office – your signature must be witnessed by a clerk.
